Per occurrence accident coverage

Simple-to-use benefits

Per occurrence accident coverage can help employees by paying for 100% of eligible services and supplies related to an accidental injury, up to the benefit limits.1 Benefits are paid regardless of any other health insurance employees may have. There is no schedule of benefits, and eligible services related to the original accident are covered even if they’re incurred on different days or with different providers.2

Customizable coverage

Groups can offer a plan that covers one or three accidents per person, per calendar year. They decide who is eligible to enroll, the number of occurrences covered in a calendar year and the per occurrence benefit amount.

Scheduled benefit accident coverage

Simple-to-use benefits

Scheduled benefit accident coverage pays a fixed benefit amount after an accidental injury, based on the type of injury or medical treatment incurred. Benefits are paid regardless of any other health insurance employees may have and can help close coverage gaps when there are deductible, copay or coinsurance requirements to meet. Symetra offers three plan options—Base, Classic and Premier. Each plan offers the same schedule of coverage, but at increasing benefit levels.

Customizable coverage

Groups can customize their coverage to ensure they have the right plan for their group. They decide who’s eligible to enroll, which plan(s) to offer and what additional coverage options to include.

Our in-house administration team, Symetra Select Benefits, handles all enrollment, billing, claims and benefit payments for our group accident policies. They're committed to delivering a positive experience for groups and their employees.

 


Per occurrence accident coverage and scheduled benefit accident coverage are insured by First Symetra National Life Insurance Company of New York, New York, NY. Mailing address: P.O. Box 34690, Seattle, WA 98124. They are not a replacement for major medical or other comprehensive coverage and does not satisfy the minimum essential coverage requirements of Affordable Care Act.

Per occurrence accident coverage is designed to pay benefits up to a preselected, per occurrence amount. It does not pay benefits for losses caused by sickness and is not intended to cover the cost of all hospital or other medical services. Policy form number is LGC-10011P/NY 10/11.

Scheduled benefit accident coverage does not pay benefits for losses caused by sickness and is not intended to cover the cost of all hospital or other medical services. Policy form number is SBC-03510/NY 1/18.

Coverage may be subject to exclusions, limitations, reductions and termination of benefit provisions.

1 Unless covered by workers' compensation or similar law.

2 Expenses must be incurred within 52 weeks from the date of the accident, and the first expense must be incurred within 60 days after the date of the accident.
