Select Benefit Administrators, an administrative division of Symetra Life Insurance Company, will handle any questions you have about your benefits, claims status or eligibility.

When can I enroll? You must enroll within 30 days of your eligibility date or you may have to wait until the next open enrollment period.
How do I use my benefits?​ Present your Select Benefits ID card to your provider at the time of service. You then have two options for filing a claim.
How do I use Select Benefits if I already have a major medical policy?​ Select Benefits does not have a coordination of benefits provision. This policy pays regardless of any other coverage you may have, enabling you to offset expenses as a result of an illness or injury. See our How to Submit a Claim page for more information.​
Whom can I contact if I have questions about my benefits? Select Benefit Administrators (SBA) at 1-800-497-3699 or Customer service representatives are available Monday through Friday, 6:30 a.m. to 5:00 p.m., Central Time.​
If I want to use my coverage and have not yet received my insurance card, what information do I need to give to my health care provider?​ Ask your employer for the Select Benefits case number before your visit. Give the number to your provider along with the SBA phone number, 1-800-497-3699.

Whom do I contact if I have a change in my name or address, or if there is an error on my Select Benefits ID card?​ Contact SBA at 1-800-497-3699 or
Can I visit my current health care provider? Yes, you may visit the provider of your choice. Select Benefits does not have required networks or preferred provider lists.​
If I have an hourly plan, how do I know which level of benefits I have?

The number of hours you work per month determines your benefits level. For example, if in September you work 96 hours, you’ll be eligible in October for Level II benefits. If you work 133 hours in October, you’ll be eligible for Level III benefits in November.

Select Benefits insurance policies are not a replacement for a major medical policy or other comprehensive coverage. They are designed to provide benefits at a preselected, fixed-dollar amount. Coverage may be subject to exclusions, limitations, reductions, and termination of benefit provisions.
