WHAT YOU SHOULD KNOW ABOUT SELECT BENEFITS
This cost-effective coverage is designed to provide benefits for routine medical events such as doctor visits, X-rays and lab tests. It pays a fixed amount for a covered benefit and can help reduce your out-of-pocket medical costs. Select Benefits isn't a major medical policy and won't cover the majority of expenses related to serious health and medical issues.
All claims and administration are handled by Select Benefit Administrators (SBA). Dedicated exclusively to Select Benefits business, SBA is available to help you understand and use your benefits. They can assist with enrollment, claims, billing and more. Please feel free to contact them with any questions or concerns.

WHAT HAPPENS ONCE YOU’RE ENROLLED
You’ll receive in the mail your ID card, policy certificate and information about how your new policy works. If you enrolled in an hourly plan, when you receive your ID card please call Select Benefit Administrators at 1-800-497-3699 to verify your coverage level.
USING YOUR BENEFITS
To use the coverage allowed by your policy, simply present your ID card to your provider at the time of service. You then have two options for filing a claim.
Select Benefits insurance policies are not a replacement for a major medical policy or other comprehensive coverage. They are designed to provide benefits at a preselected, fixed-dollar amount. Coverage may be subject to exclusions, limitations, reductions, and termination of benefit provisions. Select Benefits policies are insured by Symetra Life Insurance Company located at 777 108th Avenue NE, Suite 1200, Bellevue, WA 98004, and are not available in all U.S. states or any U.S. territory.
