  • Optional benefits include health screening benefit, recurrence benefit, buy‐up benefit and continuation of coverage.


  • Pays for eligible expenses up to a set benefit amount per accident occurrence for up to three accidents per year.

  • Benefit amounts of $300 to $10,000 per person, per occurrence are available.

Critical illness insurance policies are designed to provide benefits at a preselected, fixed dollar amount for specific critical illness conditions. Critical illness and group accident insurance policies are not a replacement for a major medical policy or other comprehensive coverage and do not satisfy the minimum essential coverage requirements of the Affordable Care Act. Coverage may be subject to exclusions, limitations, reductions and termination of benefit provisions. Critical illness and group accident insurance policies are insured by Symetra Life Insurance Company, 777 108th Avenue NE, Suite 1200, Bellevue, WA 98004. Critical illness root policy form number is SBC 00500 6/14 in most states; group accident insurance policy form number is LGC‐10011 10/11 in most states; not available in all U.S. states or any U.S. territory.
