WHAT YOU SHOULD KNOW ABOUT SELECT BENEFITS

This isn't major medical coverage. Select Benefits is designed to pay for routine medical events; it won’t cover the majority of expenses related to serious health and medical issues. It pays a fixed amount for a covered benefit and can help reduce out-of-pocket costs if you have no insurance or a high-deductible health plan.

Symetra Life Insurance Company has insured Select Benefits since 1999 and it’s been administered by their in-house administrative arm—Select Benefit Administrators (SBA)—the whole time. Whenever you call SBA with a question during business hours, you know you’ll be greeted by a knowledgeable and helpful member of their team—not a recorded message.


WHY YOU’LL LIKE IT

1. Pre-existing conditions are covered.

2. You can see the provider of your choice—no networks are required.

3. No medical underwriting, except for late entrants on life and disability income.

4. No deductibles on covered benefits.¹

5. The benefits can be assigned to your medical provider or to you, the insured.  

6. The plan pays regardless of any other insurance coverage you might have.


WHAT HAPPENS ONCE YOU’RE ENROLLED

You’ll receive in the mail your ID card, policy certificate and information about how your new policy works. If you enrolled in an hourly plan, when you receive your ID card please call Select Benefit Administrators at 1-800-497-3699 to verify your coverage level.


USING YOUR BENEFITS

To use the coverage allowed by your policy, simply present your ID card to your provider at the time of service. You then have two options for filing a claim.
