ENROLLMENT AND CLAIM FORMS
Your Symetra representative can help facilitate the enrollment process and work with you and the claimant in the event of a claim.
GROUP LIFE CONVERSION PROVISION FORM
The Conversion Right allows eligible Group Life insureds whose coverage is terminating to request to continue all, or part, of their life Basic or Supplemental insurance coverage. Benefits may continue under an individual conversion policy; no evidence of insurability is required.
SUPPLEMENTAL GROUP LIFE PORTABILITY FORMS
The Portability Continuation Privilege allows eligible insureds to continue coverage that was in-force under a Symetra Supplemental Group Life policy at group rates, as long as the portability option is included in the policy.
GROUP LIFE WAIVER OF PREMIUM FORMS
The Waiver of Premium provision allows Group Life insureds who become totally disabled to waive their premium while still continuing coverage.
